Fórum života vzniklo v roku 2001 a fungovalo ako neformálne zoskupenie jednotlivcov a organizácií. Spočiatku voľnejšia forma spolupráce bola vyhovujúca. Postupom času sa však ukázalo, že je potrebné spolupracovať intenzívnejšie a koordinovanejšie, na čo už model neformálneho zoskupenia nepostačoval.

Preto 19. decembra 2003 vzniklo občianske združenie Fórum života ako organizácia so svojou právnou subjektivitou. Dňa 17.januára 2004 sa v Rajeckých Tepliciach konal historicky 1. Snem Fóra života, o.z.
